Donna is a successful wedding planner in St. Louis. When her lifelong best friend, Cameron Ostroski, meets Andie Hune, Donna can see that Cameron has finally met the woman he will marry. Even before he sees it. She is so happy that Cameron and Andie have found each other. Donna wishes she could be as lucky. She's been planning her own wedding since she was six years old. All she's missing is the guy.
As Cameron and Andie get serious, falling deeper in love, Donna is working around the country, planning destination weddings. Exhausted, she returns to St. Louis and plans Cameron's engagement surprise for Andie. Dealing with Andie's prickly, distant mother, Caroline, and loving but meticulous father, Henry, is a chore for Donna, but she's dealt with worse. The engagement surprise is a fantastic night, and Donna plunges headlong into planning Andie and Cam's wedding day, which Donna promises Andie will be the most amazing ever...next to Donna's. She's been planning that a long time and knows what she wants for that special day down to the napkins.
But Donna gets sick through the planning. She keeps it a secret until the pain and exhaustion become unbearable. Finally going to the doctor, she's shuffled off quickly to specialists, only to find out she's got inoperable cancer that's spread. The doctors give her a short time to live. Forgoing treatment that will put her in bed, Donna vows to herself to make it through Cam and Andie's wedding. She wants to do this one special thing for her friends.
When the truth is discovered dramatically, Donna comes clean with the devastated Cameron and Andie. Donna's major concern is that they not replace her. She wants to give them this special day.
Called down to the bridal shop over some confusion with bridesmaids' dresses, Donna discovers that the wedding gown has been altered...to something that is more to her liking than Andie's. Stymied, she steps out of a dressing room to find Cameron on one knee with an engagement ring for Donna. Andie and he have decided to forgo their wedding day to give their friend the one thing they can give her before she dies....the wedding day she's dreamed of since she was a child.
Donna protests, but Andie is adamant. She wants Donna to experience walking down the aisle and marrying a man who loves her - Cameron.
But this surprise explodes, Andie's parents are furious. They want to be paid back for the expenses and for canceling their daughter's wedding. Cameron sells his prices possession, Betty Blue, his vintage blue Camaro which he restored, to Henry, to pay back what was spent on the canceled wedding. But Andie and Cameron are determined to pull this off for Donna, wanting her to feel the love they have for her.
When Donna finds out Cameron's sacrifice, she confronts Henry and bargains with him to buy back Cameron's car. She intends to give it to him as a wedding gift. She reminds Henry what a wonderful, giving, loving daughter he raised. How proud he should be that she is the person she is. It's a moment where Henry reassesses the decisions and feelings about Cameron marrying Donna.
Arriving at the church for the wedding, Donna is feeling terrible but vows to get through this day. But when she discovers the church empty, she's confused and a little sad...until she steps outside and finds the wedding had to be moved to a park across the street because so many people wanted to be a part of the day. Donna spends the day reuniting with friends, family and many people whose weddings she was part of. Donna's wedding day is far greater than anything she could have imagined.
