In our experience we have found that surgical site infection is a very troublesome chapter in our surgical practice. Laparoscopic surgery is the most popular modality of surgery now-a-days. We found a special variety of non- tubercular granulomatous infection since long at the surgical site, quite different from common surgical infection. Moreover, most often this infection is refractory to conventional dose and duration of common antibiotics. We have termed this variety of infection as ¿Micro-cluster infection¿. When this occurs as laparoscopic port, we term this as ¿Micro-cluster port site infection¿. Micro-cluster infections are reputation killing situations both for the surgeons and hospitals. This is a leading cause of hospital morbidity and patients usually suffer a lot. This not only increases the treatment cost of the patients, but also increases the cost of the hospital. Our experience suggests that this also has medico-legal aspects. So many obvious and hidden factors are related to this troublesome condition. Therefore, to manage the situation properly, we must have a guideline to reduce the magnitude of the problem.
