Whilst the greatest effort has been made to ensure the quality of this text, due to the historical nature of this content, in some rare cases there may be minor issues with legibility. In order to avoid the use of frequent and elaborate footnotes to which I am partial, with, I am told, exasperating effects upon many readers, I have on this occasion adopted the method of giving a short bibliography of references at the conclusion of most chapters. I have to express my best thanks to Mr. John H.
